As parent and/or legal guardian I remain legally responsible for any personal actions taken by the above named minor. I agree on behalf of myself, my child named herein, or our heirs, successors, and assigns, to hold harmless and defend Church of the Epiphany and the Catholic Diocese of Richmond, its employees and agents, chaperons, or representatives associated with this event from any claim arising from or in connection with my child attending the event or in connection with any illness or injury (including death) or cost of medical treatment in connection therewith, and I agree to compensate the Diocese, its employees and agents and chaperons, or representatives associated with the event for reasonable attorney’s fees and expenses which may incur in any action brought against them as a result of such injury or damage, unless such claim arises from the negligence of the Diocese.
I hereby warrant that to the best of my knowledge, my child is in good health, and I assume all responsibility for the health of my child. In the event of any emergency, I hereby give permission to transport my child to a hospital for emergency medical or surgical treatment. I wish to be advised prior to any further treatment by the hospital or doctor. In the event of an emergency, if you are unable to reach me at the above numbers I give permission for the noted emergency contact to be notified. I will not hold Church of the Epiphany and the Diocese of Richmond responsible for authorizing any medical treatment beyond necessary transportation to the hospital.

Parent/Participant Codes of Conduct
Youth participants and their parents must read, understand, agree, sign and return this sheet with the Medical Information and Release form. Each participant is expected to adhere to the following principles while at the event:
Classic Club Cotillion (CCC) is an all-volunteer, parent-organized activity. Because there is no fee associated with the events, we expect donations of food and assistance with clean-up at the end of the evenings.
Attendance at CCC is a privilege. All inappropriate behaviors could result in being barred from CCC.
All participants will adhere to the Dress Codes for Ladies and Gentlemen as noted in the Cotillion Handbook.
All social dances will be taught by a trained, background screened, professional dance instructor. All social dances will instruct participants to physically touch their dance partners in an appropriate and professional manner.
All participants will be required to rotate through many partners. Participants are strictly prohibited to break in and out of line to avoid specific partners. This behavior will not be tolerated.
Participants will remain respectful and quiet when the dance instructor is teaching or when an adult is giving instruction and/or direction.
Participants attending the CCC must engage fully in all activities.
During CCC events, gentlemen will be given instruction to escort ladies to their seats and assist with refreshments.
Signing in, making a nametag, making good eye contact, greeting and shaking hands with all participants and adults, using manners, and learning morality lessons are keys to the success of CCC events. Please encourage all participants to work on the skills they learn at CCC in the “real world.”
For any violations of the above-mentioned codes of conduct and for any behavior judged as inappropriate by adults at the event: Sarah Brockwell, Director of Christian Formation and Parent Coordinator, Stephanie Eggleston, have permission to discuss the infraction with the participant. If the issue cannot be resolved by discussion, parents will be contacted.
I have read, understand, and agree to the above principles, the dress codes and the expected behaviors of a CCC participant. Any violation of the above principles may result in immediate dismissal from the event. We agree to all terms and conditions.
